1. Connect
to the Internet. An Internet connection is required because
only the form data is stored on your computer, not the actual
form itself. Your computer will retrieve a blank form from
the IAR server and automatically fill in the fields using the
file you have saved on your computer.
2. Locate the saved
form file on your computer.
3. Open
the saved form file by double-clicking the icon.

4. Adobe
Reader will open and display your completed form. You may be
prompted for a username and password to access the form. Use
your last name as the username and the last
four digits of
your social security number as the password.

5. Once
the completed form is loaded, you may update fields as necessary,
re-print the form, and re-save the form.
